refactor: split complex functions into helpers (S3776)
Lower the cognitive complexity of paragraph, axis_scale, modify_ref_list, fill and reconstruct by moving their per-run, per-axis-limit, per-entry, per-request and per-block steps into helpers. The expressions and their evaluation order are unchanged.
