3 people 73bf2a47b5 Layout fixes: mirrored margins, tall headers/footers push the body, endnote continuation, footnote caret on continuation pages (#452, #453, #454, #456, #457) (#490) ...

- Mirrored margins: even pages put the inside margin and gutter on the right
(text, notes, header and footer shift; page borders stay).
- A footer taller than the bottom margin moves the body bottom up.
- Headers and footers inherited via link-to-previous reserve their room in
later sections (shared `linked` helper with the drawing pass).
- Long endnotes split at line boundaries and continue in the next column or
  page (bounded by MAX_NOTE_PAGES).
- Caret/selection lookup in a note collects the paragraph's pieces on the
neighbouring continuation pages, not only the hinted page.
