 a2d9a9480c M1: validate object-number ranges before reading cross references (#92) 
Reject invalid xref ranges before arithmetic or narrowing casts, allowing the existing reconstruction path to preserve intact objects and record the repair reason. Cover both tables and streams through Document::open.
