Eric Kaden c392d93ba6 M12: laying out a dynamic XFA form takes off the usage rights it voids ...

The layout revision replaces the placeholder pages and adds the fields,
so usage rights (/Perms /UR3) signed over the form as published no
longer match it, and a viewer may then open the form with every field
locked. write_form now takes them off with the helper strip already
needed, and the layout report says so.
