aiongg fcedb15689 fix(export): find font size and colour slots through a map ...

The body-size tally and the RTF colour table searched every slot seen so
far for each block, and the RTF writer searched the colour table again for
every paragraph and cell, so a document with many distinct sizes or colours
made export quadratic. Both keep their first-seen order and look slots up
in a hash map.
